Our fees are based on the quality of the service provided.  We encourage you to ask any questions you may have before your evaluation begins.

Payment, in the form of check or cash, for services is expected at the time of the appointment.  We will be happy to submit claims to your insurance carrier for covered services. 

It is your responsibility to understand the type of coverage your insurance company provides.  Please have any necessary referrals sent to our office by your primary care physician before your scheduled appointment.
